Emergency? Call 911 first.

EMPLOYEE REPORTS

Property, vehicle, equipment, product, technology, or client-site incidents with no injury.

Any work-related employee injury, even minor injuries.

Complete after reviewing the incident with the employee.

Complete after reviewing the injury with the employee.
